At Christ Community Church, your child's saftey is one of our biggest concerns. We have created liability waivers/parental consent forms so that you (the parent) are aware of what your child will be doing at events and the risks involved. Below are the liability forms. Print out the form or forms that you need. Read them carefully and fill in the form. Some of the forms need a notary to stamp and sign. DO NOT SIGN THEM UNTIL YOU ARE PRESENT WITH A NOTARY. Christ Community Church has a notary on staff that would be willing to help. When completed, please return them to CCC's office at your earliest convenience prior to the event.
